Our Services:
Gift Permanent Supportive Housing, Inc. is a permanent supportive housing facility, which consist of spacious two bedroom apartments, two people in each apartment
. > We provide three meals a day; breakfast, lunch and dinner
> Our clients meet weekly with the Director to discuss their progress in our housing program. If it is determined that a client needs professional counseling or treatments, referral is made to assist the client with his/her issues or problems. All clients are given sufficient opportunities to participate in various community services, based on their individual needs.
> We provide free usable clothing on admission, and subsequently, if needed, and is available. We also provide a personal hygiene care package, with all of the basic accessories.
> We provide transportation arrangements are made for medical and legal appointments, via southeast trans, or with family members. If the client is able to ride public transportation, the client buddy system will be provided.
> We provide Intensive Case Management services, Peer support-Health and Wellness program, GED assistance, pre-employment and job readiness counseling, HIV Education/Prevention/Testing, and HIV Targeted Case management, budgeting and money management skills. Periodically, each client is evaluated for independent living readiness, if they meet the standardized requirements, the individual will be assisted with placement within our housing complex, with options provided within the larger community
